Successful hospices operate with clear, transparent operational metrics and goals. This daily tracker is a dashboard, and the critical point is to bring visibility at the end of the day to what transpired during the day. These numbers and simple calculations are a daily, weekly, and monthly view of your hospice's performance. For example, when a revocation occurs, perhaps a series of questions or inquiries should take place. Should have met or addressed fundamental misunderstandings or concerns, provided inadequate services, or met the patient and family needs? What actions are now needed with this patient and their family? These types of questions should be triggered by seeing data on the daily end-of-day report. Hospice was established so that each patient and their family can experience a good death; when we see deaths at the end of the day, do we ask ourselves if we have completed our mission and celebrate the staff that provided this wholistic experience, or do we like many hospices worry simply about the census goal?
